execute external comand with challenge-response mode

I would like to use python-pam to execute an external command (some custom check) and evaluate the error code to validate the pam stack.

I'm creating a custom script for TOTP validation and I need a challenge-response (Introduce PIN) and then execute an external command. Similar to pam_exec, but pam_exec don't support challenge-response.

Is there any work done similar to the setup I'm describing?
