Update from code changes: knowledge document scoping and always-include toggle

Summary

Updates the AI agents knowledge documents guide to cover the new Always include in context toggle, the status chip in the documents table, destructive-action confirmations, and how documents are now scoped to a project and agent.

Changes

• Clarify on-demand retrieval as the default behavior for knowledge documents.
• Add an Always include in context subsection covering the toggle, when to use it, the token/latency trade-off, and the confirmation flow (confirm when enabling, immediate when switching back or when deleting).
• Note the new Always included / On demand status chip.
• Explain that documents are now listed per agent — an agent only sees org-level documents plus the ones granted to it — and update the per-agent scoping tip.
