Summary

Each saved hook run can now be seen in one table.

The table shows how often each hook ran, what happened, how long it took, and its last error.

A short prompt message points out new hook failures in Claude, Cursor, and Codex, without stopping the prompt.

Overview
Adds on-demand visibility into hook runs recorded by the existing metrics runner, plus a non-blocking prompt nudge when new failures show up since the last message.

engine/hooks/_runner/report.py compares currently registered catstack hook commands (Claude/Cursor/Codex configs) against runs.jsonl for a --since window. It prints a per-hook table (or --json) with outcome counts, p95 latency, and last error; registered hooks with no rows show no record (not “healthy”); stray log rows appear under unregistered:; bad lines and unreadable config/log surface as warnings or exit 2.

New advisory hook-health hook (Claude/Codex UserPromptSubmit, Cursor beforeSubmitPrompt) tails the same metrics log from a per-session byte offset, and injects harness-specific additional context when new rows for that harness are crashed, timed_out, or caught_error (ignores itself; missing log stays silent; unreadable log says failures are unchecked). Notices point agents at report.py and never block submission.

Install/docs/tests: install.sh symlinks and merges hook-health for all three harnesses; docs/ecosystem.md lists it; unit tests cover the report CLI, detect/runtime behavior, and install wiring.

…ooks/_runner/run.py runs one hook script, passes its stdout, stderr and exit code through unchanged, and appends one metrics row with a classified outcome.

datetime.UTC exists only on 3.11+, so on CI's Python 3.9 the runner raised
after the hook ran and dropped the hook's stdout and exit code. Use
datetime.timezone.utc, and catch any failure while classifying or writing the
metrics row so the hook's output is still forwarded, with one stderr line
naming the error.

Once wrap_installed.py rewrites an entry to `_runner/run.py --timeout T
<hook>/<script>.py`, the only $HOME/.claude/hooks/ path in the command is the
runner itself, which exists, so a deleted hook's entry was never pruned.
prune_dead_hook_entries.py now also checks the hook script named after the
runner.
